U.S. Vital or Civil Records. How to get them
The Embassy or Consulate can’t obtain vital records on your behalf. Vital records include but are not limited to a birth or marriage certificate, a certified copy of a divorce decree, etc.
In the U.S., state, county or city governments, depending on location, usually hold such records. Please follow the link to The National Center for Health Statistics for more information on obtaining vital records.
Note: For your U.S. document to be accepted by the Dutch authorities, for example if you plan to get married, you must provide your document with an apostille. Please see our section on apostilles for further instructions.

TAMPA, FL – (April 17, 2014) – The Hillsborough County
Tax Collector’s office recently partnered with the Florida Department of Health’s Bureau of Vital Statistics, to allow Florida-born citizens to get a certified copy of their birth certificate at the Tax Collector’s office. This partnership will help make the process much easier since a birth certificate is one of the items needed to obtain a driver license or identification card. “If a customer does not have their birth certificate, we don’t have to send them away to get
it. We’ll be able to issue them a certified copy on the spot.” Belden stated. Birth certificates are currently being issued at the North Tampa office located at 3011 University Center Dr. Suite 150, in the Temple Terrace area. Certified copies of FL birth certificates will be offered in our other full service branches by the end of September. To get a certified copy of a FL birth certificate, customers will need to present a valid form of identification such as a driver license,
Florida identification card, passport, military ID or any other acceptable government issued identification. The cost of a certified birth certificate is $20.25. There is a $9.00 fee for any additional certificates issued for the same person processed at the same time. Florida birth certificates may still be obtained through the Bureau of Vital Statistics and the Hillsborough County Health Department. much does a vital record copy cost from Florida?
Vital Records Online will charge a fee starting from $49 for the assisted service whenever you order birth records, marriage certificates or licenses, divorce certificates, or death records. The state of Florida will charge a $9.00 fee for a certified copy of a birth certificate. There is a $5.00 fee for a marriage certificate copy and a $5.00 for a death certificate. Any additional copy will cost $4.
