Birth records start at 1878 to current and are available at the Buffalo City Clerk's Office located on the 13th floor of City Hall in room 1302. All Birth Certificates issued are certified with an official raised seal. WHO IS ELIGIBLE TO OBTAIN A BIRTH CERTIFICATE COPY?

  • The person named on the birth certificate.
  • A parent of the person named on the birth certificate (requesting parent's name must be listed on the birth certificate)
  • Only by order of a New York State Court or a Government agency for a legal or proper purpose may a spouse, child or other persons obtain a copy of a birth certificate.
  • If you have custody of a minor under the age of 18, you will need custody papers that are no older than 6 months otherwise you must obtain an updated copy from the court.

IDENTIFICATION REQUIREMENTS - application must be submitted with copies of EITHER A or B:

Two Ways to Get a Birth Certificate in New York

The Old-Fashioned Way:

Learning how to obtain certified copies of New York birth certificates isn't difficult, but

 there are a few things you need to know. There are two ordering procedures: one for documents relating to persons born in New York City, and one for persons born elsewhere in the state.

1. For birth certificates in New York City (Brooklyn, the Bronx, Manhattan, Queens, or Staten Island):Print out the online application (PDF) provided by the New York City Department of Health and Mental Hygiene. Fill it out and have it notarized.

You must also send a photocopy of one of the following documents:

  • Driver’s license or non-driver’s ID from any U.S. state or territory
  • U.S. or foreign passport
  • U.S. certificate of naturalization
  • Military ID card
  • Public benefit card
  • Employee ID w/ photo, verifiable employer, & recent paystub
  • Student ID and current transcript from accredited, verifiable institution
  • Inmate photo ID w/ release papers
  • MTA reduced-fare Metro Card
The document must be unexpired and bear your photo as well as your signature

. If you do not have any of the above documents, you can send along proof of address (either a utility bill or a letter from a government agency).

2. For New York state birth certificates outside NYC:To order a non-NYC New York birth certificate (i.e., outside the five boroughs of New York City), you must be either (A) the person named on the document or (B) a parent listed on the document. Anyone else must have permission from a New York State Court.
