With the three OMNIA options — OMNIA Bronze, OMNIA Silver, and OMNIA Gold — there’s no requirement to select a Primary Care Physician, or referrals needed to see participating specialists. You’ll also benefit from lower copayments and deductibles (and in some cases, no deductibles), when using OMNIA Tier 1-designated doctors, hospitals and other health care professionals.
BRONZE
- Offers lowest monthly premium but highest out-of-pocket costs
